Here's what I did and got this within 5 mins.

Difficulty, innings, teams etc. don't matter because you will be using two controllers. Just make sure Manual Fielding is on and Mercy Rule is off.

Start a Pick Up game and let Player 2 score at least 10 runs. Then strike out 3 batters with Player 1.

Once the sides are switched bunt the ball with Player 1. When the next batter is up have the previous hitter steal all the way to home. You should now have 3 power-ups. Keep bunting til the bases are loaded. When that's done, hit LB with player one and because Player 2 is so far ahead you are almost guaranteed to get a Meteor Ball power-up. If you don't get it strike out and try again. Throw a perfect pitch with Player 2, Press the left stick up and hit B with Player one, and this achievement is all yours!

Hit a Home Run with a player on each of the three bases. This may come naturally in story mode but could be easily set up in a two-player game. Walk three batters to load the bases, then throw one down the middle.

This achievement refers to Lou Gehrig, who has hit more grand slams (23) than any player in baseball history.
