At the very begining of the game where you are stuck in the reverse bear trap, do not do the puzzle to escape, rather just leave the controller and let it kill you. Achievement unlocked

Nothing complicated about this, you'll die at some point either in a trap or from a minion.

To get this quickly, just do nothing at the beginning of the game when you're in the Reverse Bear Trap puzzle and watch your head explode. You'll most likely die a bunch during the game though.

You can deliberately fail the task of freeing the main character from a bear trap, wait until the trap kills you and the achievement is yours.
