As the description states, the achievement is unlocked when you change visual equipment. Visual equipment can be an outfit or an attachment. Press 
to open the main menu and 
or 
to tab to Equipment. Under Equipment, you have equipment itself and, by pressing 
, you can tab to visuals. Equipping a non-add-on attachment or outfit will unlock the achievement.
If you have add-on outfits, you can unlock the achievement as soon as you can open the menu by pressing. Although the achievement states it does not include add-on, you can still unlock the achievement. First, equip the add-on outfit. Exit. Open the menu again and revert to the original outfit. The original outfit is not an add-on. Changing back into it counts as changing your Visual Equipment and the achievement will pop.
If you don't have add-on outfits, the earliest you can unlock the achievement is when you first arrive at your hideout. The shop in the hideout, in contrast to shops you encountered earlier, offers visuals in its inventory. They are expensive and you may not have enough cash. If so, the shop also offers exchanges. Acquire any visual and equip it. The achievement will pop. If you acquire the visual through an exchange, you will also unlock:

This achievement unlocked for me by itself. I'm not sure if it was Bugged + or if because of *spoilers free* a certain character automatically started to wear a visual piece of facial gear. But certain character had worn it before it unlocked so idk. Probably was a bug cuz it unlocked when I fell to my death.

My guess would be that, as apparel changed due to story progression, something happened to trigger the achievement. Just a guess. Interesting. Kind of like an easter egg.

The save/shop vendor will be selling cosmetic items under [visuals]. You can purchase the cheapest items for around 3000 kin.
You will then need to apply it under [Equipment] then [Visuals] at the plug in sockets.
Friendly note that only one cosmetic item needs to be purchased to equip it on yourself and your companions, so don’t buy multiple of the same one!
