This is an extremely difficult achievement to unlock as many of the 5 Star scores require near perfect timing to accomplish. As such, it's almost impossible to make a fully re-creatable solution.
I have made a series of videos showing my 5 star scores and a lot of the misc. challenges required, so that hopefully if you are struggling with a particular level maybe my gameplay can give you an idea of what needs to be done differently.
