On story mode, after the first boss (Almirant) you have two path choices, North and East. Following East path, the second stage will be a series of screen with farms.
On the last of them, you will see a caption of a mob hanging a witch on a tree. Something will happen (just avoiding spoilers here), and then the game will prompt you two options, "Fight the witch?" [Yes] / [No]
YOU HAVE TO pick [No] to unlock this and, also you will unlock the "Black Cat" minion card too, which will work you toward another achievement:

Just to be clear, you go east after Almirant, and then east again with the second choice.
