How to unlock the Guardian Ape Immortality Severed achievement in Sekiro: Shadows Die Twice - GOTY Edition - Definitive Guide
He spawns at ashina depths, right after the poison pool behind the spawning point of the snake eyes miniboss. The arena is rather large so you can't miss it.
To get him to spawn you need to first kill the guardian ape. After killing genichiro head straight to gun fort. You need to get the key from kuros room to enter the area where the guardian ape is.
After killing the ape you need to go to senpu temple and get the mortal blade. Do not go to ashina depths without the blade, you can't unlock the achievement without it.
Once you're done with senpu temple and have the mortal blade, head straight to ashina depths. Kill the snake eyes miniboss, fill your flasks and get ready for an annoying fight.
The boss is the headless ape, it's the same boss from before. Don't worry, he has less overall health this time so your attacks hit a lot harder.
You can eat akos sugar before you start the fight. Run towards him as fast as possible, you should be able to hit him 3-4 times before he has a chance to react. Run away after you attacked him, his terror howl does a lot of damage and his other attacks are hard to read.
He basically has 3 attacks.
1. He leaps towards you and does a sweeping attack. Jump towards him and try to attack him twice. Run away immidiately because his followup attack is the terror howl 90% of the time
2. He does his one-two thrust attack. I usually just run away while jumping, you can try to deflect but he has a followup that does huge amounts of damage. Not worth it.
3. He leaps into the sky and does a sweep followed by a downward slash. Watch out for this move. Jump into the air as soon as you see him land. This attack is very easy to dodge. do not run away from the downward slash. Deflect it. Successful deflect of the slash stuns him for 3 seconds. This gives you enough time to get decent damage in.
4. He slashes in front and then behind him. I never bothered to deflect here. Simply run away.
5. Terror howl. Run away.
Only attack him after he does the leap (1) and after stunning him (3)
3. Is very important. You can deal massive amounts of damage while he is stunned. Look at youtube videos if you want the right timing.
Sadly, after you wipe out his first health bar he calls for his wife. The moveset is the same as guardian ape phase 1. Luckily she has less health and is not as aggressive as the guardian ape. Use firecrackers (regular or long) to stun her and chip her down.
You will need a little bit of luck for this as fighting both apes at the same time can be a bit frustrating.

Do you have to do anything special to obtain this or do you just have to win the fight...? IE... do you have to activate the mortal blade as a final move or anything like that?

To get the achievement you have to first get the Mortal Blade from the Divine Child (it's story related so I'm sure you can't miss it) then kill both Apes. Once the headless one is defeated, go up close to the ape and swing at him again until you get the deathblow. Sekiro will kill the ape and the parasite completely, giving you the achievement after and a new Ninjutstu technique along with it.

In this video guide I will show you how to defeat the Guardian Ape's undying form. This will be available right after you beat the guardian ape for the first time in the Sunken Valley. Now go the the Poison Pool and start heading towards the back room like you were to advance and also where one on the Snake Eyed mini bosses is. Two useful things to use are the whirlwind slash and some kind of shuriken. Once you head into the room you will see him in the far back room up against the wall. Now his attack patterns are all basically the same as before. Only real new move he has is he jumps in the air, does a swipe attack, and slams the sword on the ground. Now at that point is a good time to attack him and another time is when he does those four swipes and moves like a snake while doing so. There might be time to get a hit in when he does that slow swipe attack but other than that just wait the other two attacks out to be safe. Once you got his first deathblow he will summon his wife ape and you need to deal with her first. If you think you have a clear shot on the male ape while doing one of his open attack while the wife just sits there, go ahead. Now this is where you will use your kuni or shuriken on the wife ape, use them all on her to weaken her as much as possible. Now she will attack but not very much, just attack her from time to time when she is in the back doing nothing. Just make sure the male ape isn't attacking when doing this. Now the wife ape will attack with some heavy damage as well so be careful when she attacks. She will move pretty slow when attacking except when she jumps across the cave. Now just keep on her until she is dead, she shouldn't take much more damage after using all of your throwing weapons. Once it's just you and the male ape again just wait for the openings I mentioned before and he will fall. Now once he is on the ground make sure you hit the attack button to use the mortal blade to sever the immortality and get the achievement.
