How to unlock the Purification achievement in Sekiro: Shadows Die Twice - GOTY Edition - Definitive Guide
The steps to get the Purification ending are as follows.
Firstly progress through the game until you confront Owl on top of Ashina Castle. You must choose to 'Break the Iron Code, stay loyal to Kuro'. This will trigger a boss fight against Owl. Defeat him.
The next step is to eavesdrop on Kuro and Emma in Kuro's Room by leaning up against the wall out of sight. Then hit to eavesdrop. If the prompt to eavesdrop doesn't appear, make sure you have exhausted all dialogue with Kuro and have rested at the idol.
Next speak to Emma upstairs and agree with her. Then rest and speak to her again. Next, travel to the 'Old Grave' idol, go through the door immediately to your left to speak to Emma again. Now travel to the Dilapidated Temple and eavesdrop on Emma and the Sculptor from behind the temple. Speak to Emma, and exhaust all dialogue with her to receive a 'Father's Bell Charm'.
Now interact with the Buddha Statue to be transported to an alternate version of Hirata Estate. You will start in front of the burning estate. Move forward to encounter a mini boss. You can easily kill him by kiting him back towards the bridge. He won't attack and you can Whirlwind Slash him to death.
Move past the idol and you will come to the open area where Juzou the Drunkard was originally. He is here again. Defeat him again, then proceed into the building and rest at the idol.
Now head into the arena where Lady Butterfly was to fight Owl again, but this time the fight is much tougher. Beat him to receive the 'Aromatic Flower' and an achievement.
Now just play up to the end of the game, defeat Sword Saint Isshin Ashina, and choose to give Kuro the Divine Dragon's Tears + Everblossom to pick up this achievement.
There is also a method to get this and the other two ending achievements without having to play through the game three times. Once you defeat Sword Saint, save and quit back to the dash and then reenter the game again. Choose to give Kuro the items for the ending you need, then once the achievement pops (when the credits start), hit Guide, press on Sekiro, choose Manage Game & Add-Ons, scroll down to Saved Data, and choose your saved data, then Delete from Console. Repeat until your data is gone. Now restart the game and you will be able to choose again. Repeat for the other ending to get all three.

I don't mean to come off as a douche here lol but seeing people in general struggling to do the save exploit or somehow end up in "NG+" really is baffling to me when the whole process is so simple. Like, how could you possible screw it up?
Think of it like this:
On Xbox, your saves are both local and on the cloud. Your games do not load cloud saves, the cloud save is saved onto your Xbox and the game then loads the local save. The cloud save always puts in work to be overwritten by the local save, and also overwrite the local save if it sees that there is a mismatch.
So, if you beat Isshin and saved at the idol just before Kuro, ask yourself this very simple question:
With your Cloud Save now being at the post isshin idol, how do I go about unlocking an ending achievement and ensure my cloud save, which is usually overwritten by the local save, isn't overwritten?
The answer is extremely simple: Cutoff your internet connection. Do not give the console an opportunity to overwrite the cloud save with the newer local save.
With your post isshin idol save, remove your internet connection (bonus: also go offline on your xbox's network settings), do the deed (ideally, watch the credits for 30 seconds), quit the game, delete the local save through Manage Games and Add-ons, plug your internet back in, relaunch the game to resync your post isshin idol save to the console, repeat until you get all endings.
I had zero issues save scumming using this method, and it makes sense. Xbox always tries to sync saves to the cloud. Simply cut off that pipe to make the exploit accessible.
The same trick would work for the all skills achievement. Cut off your internet - spend all you points, get the achievement, quit the game, delete the save, plug internet back in, resync old save. Now enjoy spending all your skill points in extra attack power.
NOTE: Keep in mind, when you earn an achievement offline, it will not pop until you are back online.

Doesn't "delete all" delete local AND cloud save data? Don't you want to just delete the local save so you can resync with the cloud?
