How to unlock the Eyes Of The Storm achievement in Shadow of the Tomb Raider - Definitive Guide

All three of these are rewards for completing the "The Age Makers" side mission that comes the DLC, so you cannot unlock this achievement without first unlocking

Once you have completed the side mission, go to a base camp and equip the Oathtaker's Bow - Eternal Sentinel bow and the Sinchi Chiqa Battle Dress - Honor Guard outfit. Note that both of these are variations on similarly named items that were bundled with the DLC, so be sure that you are equipping these exact items, not their more vanilla counterparts. You learned the A Thousand Eyes skill at the challenge tomb, so you don't have to do anything at the base camp to make it happen. If you need to check this, press cn_X from the Skills menu at the base camp.

EDIT: Fast travel to a base camp outside of the Hidden City to equip the outfit.

Once you have equipped the bow and outfit and exit the base camp, the achievement will unlock.

The game glitched literally RIGHT when I was awarded the achievement causing me to do a hard reset. Fortunately, it still counted.

is there a specific base camp?
I have selected all the exact elements indicated and it does not unlock me anything ...
I wouldn't want him to be buggy

I found that if I went to the Tomb challenges, from the main menu, and selected the tomb that the clothing was achieved in, that you could equip it. And at the same time equip the necessary weapon for the achievement. Then go to start the challenge- achievement pops. Then exit to lobby. Rinse and repeat.
I don't think you need to change this tomb to equip the other special versions of the outfits, they all seemed to be available to equip.

I equipped both the bow and outfit at the campsite inside skull cave, right after I completed the quest and achievement popped No need to fast travel anywhere.

In order to obtain the skill, you need to complete the "Path of Huracan Challenge Tomb". Also, you need to equip both elite versions of Oathtaker's Bow and Sinchi Chiqa Battle Dress, named Oathtaker's Bow - Eternal Sentinel and Sinchi ChinelBattle Dress - Honor Guard. Those 2 are obtained after completing The Age Makers Side Mission (after your final talk with Q'orianka).

The mission location is inside Skull Cave (there is a Base Camp with the same name). The mission starts after talking with Q'orianka.

Tiny correction: As pointed out in the other solution, the outfit's name is Sinchi Chiqa Battle Dress - Honor Guard. The bow is Eternal Sentinel. The skill is obtained automatically while completing the tomb.

After you complete The Age Makers side mission, you will have unlocked the A Thousand Eyes Skill and received the elite versions of the Oathtaker's Bow and Sinchi Chiqa Battle Dress. Head to the nearest fire and equip both of these (make sure you equip the special versions and not the "normal" versions) to unlock the achievement.

Once you have completed the new Challenge Tomb in "The Age Makers" side mission, you will automatically unlock the A Thousand Eyes Skill. Then, once you complete the side mission entirely, you will be rewarded the Oathtaker's Bow - Eternal Sentinel and Sinchi Chiqa Battle Dress - Honor Guard.

All you need to do now is head to any base camp outside of Paititi and equip the aforementioned weapon and outfit. The achievement will unlock when you back out of the base camp and regain control of Lara.

Battle suit "Sinchi-Chiku" - Bodyguard.
