How to unlock the Put Herself Together achievement in Shadow of the Tomb Raider - Definitive Guide
The "Mother Protector" Challenge Tomb is a later part of "The Path Home" DLC. To access it you'll play through the new mission. The entirety is shown in my Walkthrough below with a run time of 27 minutes. The specific tomb challenge is around 11:35 in the video.
here is how to solve the Mother Protector puzzle:

1) start by rotating middle lever to open side doors. go through the Right door first. move leg out of the way
2) slide down to the middle. rotate the center lever again to raise the center switch. pull the switch once, to move the arm holding the hammer to the right.
3) rotate the center lever again to reopen the side doors
4) since the block moved down you can now climb up the right wall back to the top and position the leg first, then the arm
5) slide down and access the left door. go up and solve the puzzle by moving the leg first (switch is already set to bottom). then the top arm holding the dagger
6) slide down back to the middle of the room and progress over the bridge
