Shadow Warrior 2

I obtained this achievement by putting the follwing upgrade gems into my "Bladez of Zaibatzsu", and then just hacking & slashing away at enemies:

- Frosty Soul (+32% chilling effect / +15% chilling effect duration / +10% extra damage as ice)

and

- Essence of Ice (+13% chance to freeze / +18% damage converted to ice)

Both of the above upgrades were of "orange rarity", I'm not certain which actual in-game rarity that equals.

My third upgrade was "Stinging Aspect of Threat" (+64% critical damage / +11% damage to Superior and Elite enemies), which was of "blue" rarity, but did not add any specific ice damage to my blades. The more ice-related upgrade gems you apply to one weapon, the better your chances are at dealing ice damage and/or freezing enmies, of course.

This achievement can be easily combined with the "Trickster" achievement (Perform 200 Special Attack kills), by using the Vortex special move ( or followed by ) on groups of enemies.
