Story related - you cannot miss this achievement through natural play through.
The objective of this part of the story is to activate all three mechanisms in order to power the device used to lure the dragon through “magic.”
Tip: For each mechanism, if you cannot find the switches, follow the purple tubes stemming from each cannon. They will lead you to each switch.

I've finished the game and it didn't unlock for me for some reason...
It unlocked the second time I finished the level.

In the event that any of the story-based achievements do not unlock, using Chapter Select appears to work. My first playthrough did not unlock the achievement for the 'Midnight Snack' level, but unlocked after I replayed it.
@FlawlessPine483, did you use chapter select or did you replay the entire game? I am hearing mixed reports on whether these bugged achievements can unlock via chapter select.
