How to unlock the Headache achievement in Shadow Warrior - Definitive Guide
The demon head refers to the large axe dual-wielding Demon.
There is a skill that allows you to pick up their heads, but I am not sure if you have to learn it because you can start picking up the heads between Chapters 8-9(will clarify in edit). Once you obtain a demon head, select it and use RT. It will shoot out demonic beams. Direct the beams at lower demons and it will kill them pretty quickly.

Yes, there is a skill that you manually unlock in order to do this.

To be able to obtain a Demon Head as a weapon you must first purchase Headhunter I skill under the Knowledge tab. Headhunter II can also be purchased to increase the Demon Head damage by 30% but isn't required for this achievement.
If you haven't obtained 50 kills using Demon Heads while you made your way through the campaign then try and save one in Chapter 16. You'll have a chance to grab one in the big fight before you face Zilla. The reason you're holding on to it is because at the very start of Chapter 17 you'll be on a long linear stone pathway that has about 30-40 skeletons and easy to kill demons. Make a save as soon as the cutscene ends (you should get a checkpoint too) and then start running up the stone path. Once a large number of skeletons and demons have grouped up pull out your Demon Head and slaughter them all. If you don't get it on the first go, reload your save and you should be done with it quickly.
This is also a great spot to get other achievements that require kill counts with various methods and weapons.

You need to unlock the headhunter skill with one karma point to be able to pick up demon heads, which will drop from each Warlord that you kill. Select it from your weapon wheel and use it to kill as many enemies as you can before the 'ammo' runs out, then pick up the next one you encounter and repeat the process until you kill 50 enemies with them.
