I got this achievement without trying while playing through the story. I don't think you can get a kill without getting a dismemberment too. The wild swinging of the sword and constant action during fights results in regular decapitations and dismemberments. Without really trying for it, I unlocked this achievement during Chapter 3. Unlock decapitations, you can dismember enemies with the guns too, so either play style really works for this achievement.

You can easily dismember enemies with your Katana. This should come naturally without any need to boost it within the first few chapters if you use the sword continuously.
