I believe this can be done right at the beginning of the game with a new character. It requires 11 Karma if starting from scratch. DE4N MUFC noted, you can start as the Shaman archetype to give you a boost. I believe starting as an elf may also give you a boost.
Simply spend your karma in the yellow "CH" section.
-Level you Charisma to 3 using 5 Karma.
-Then level your Spirit summoning to 3 using 6 Karma
-Then select any of the spirit totems. And hit
to confirm.
Achievement should unlock =)

It should be said that when at the Create Your Character screen you need to choose the Shaman option under Archetype. You only start with 8 Karma but choosing Shaman automatically levels up your charisma thus allowing you to have enough Karma to unlock the totem spirit.
