This achievement is incredibly simple to get but might come with some confusion. When the game starts you'll be sent to a flashback sequence. This is not where you get this achievement. Complete the flashback sequence and continue progressing through the story.
Eventually you'll meet a character named Jake Armitage who will ask for some help in dealing with some Halloweeners (in the lore, they're a street gang that dresses up as monsters, goblins, and other ~ scary ~ things to terrorize local citizens). You will leave the building that you meet Jake in and be directed to his "stash" a short distance away. Once you get to his stash save your game before interacting with it. Move the garbage to the side, then activate the yellow/gold icon to interact with his stash. Once you do this combat will immediately start with your group (you and Jake) and two gangers.
During this encounter, every time its your team's turn, hold down the
button to skip your team's turn. Skipping your turn will allow your team to stay stationary and not fight back while the two gangers whittle away at your health. Continue doing this until your character dies. When your teammates die, they simply go into a "downed" state and can be revived. Once you die, however, the achievement should pop for you.
