How to unlock the Wetwork achievement in Shadowrun Returns - Definitive Guide

First, you need to get one character with Cleave unlocked (Strength [5] --> Close Combat [5] --> Melee Weapons [5]).

Instead of trying to get two low health enemies adjacent to perform the Cleave on both, simply get one single enemy to a very low health, position one companion beside the enemy (not diagonally) and do the Cleave on both. as long as BOTH characters get hit and the enemy dies, the achievement will unlock.

This same method works with the Killing Hands achievement where you have to kill two adjacent enemies with a single unarmed attack.

I also had to kill both targets with one swing. I just got two of my teammates really low and put them on both sides of a low health enemy in case I missed one target. There is also a teammate you get late in the game who has this skill so you don't need to dump the points on your character unless you want it early.

Just confirming I had to kill both targets. I tried the above method but it did not work.

So, I couldn't get this to work by killing a low-health enemy near a companion, so I used grenades to weaken a teammate to critical health and then moved them near the enemy.

However, when I went to attack both of them, the ally died but the enemy resisted the attack and was left with a small amount of health, but the achievement still unlocked.

I hit both characters, killing the enemy and wounding my companion, but no achievement. Do I have to kill the companion as well?

I got both achievements without killing the adjacent companion. Just hitting him/her. Only the enemy has to die

Same issue as the other commentor, I've done as described in the guide five times now to no avail, even though it worked for Killing Hands. Hopefully this unlocks in the future but something is up with it.

Same issue as Faustus & Pink Freud.
No pop for me as I only hit my team-mate and killed the enemy.
But as i killed both, the achievement popped up.
Some AoE things (burning spells, grenades ) reduces the life of team-mates

i found it a little finicky but i also got the achievement without killing both.. that being said.. you can always just set it up to kill both if you are having trouble as that seems to be working for most people
