. I did not get this achievement in 2 playthroughs, I only had around 6000 swaps. If you intend to do a prepatch playthrough with the disc version so you can get the Got Gud and Oldschool achievements (they cannot be unlocked on the current patch) I would suggest doing the rest of your swaps before starting that playthrough just to be safe. As for how to actually get the achievement, remove 2 puppets so you only have one puppet and the Devourer, and then mash or 1000s of times to get this. Very tedious.

Why does a 3rd playthrough have to be done offline?
Why bother explaining about an offline playthrough if it has nothing to do with this achievement?

Some achievements can only be acquired with the prepatch disc version, which is why a playthrough has to be done offline. I'll edit the solution to make things a bit more clear.
