Catacombs is missable.
You must either complete the spider related side quest chain in Thole (Ch.1), or tell Zaar about the secret underground pathway at the tribal hearing (Ch.2) to gain access to Catacombs. If you haven't gone there by the start of Ch.3, you'll have to get this achvt on a new playthrough.
