Start a game with custom settings - Hardest, 5 lives and 4 continues, now just let every live die and use all your continues - 5x4 = 20 deaths! Easy.

Ive done this a billion times It wont unlock! It's glitched on my tag and idk what to do...

Here is a way to fix the achievement if it is glitched on your tag.
Put your gamertag on an USB-Flash and start the game. When you see the SEGA Logo pull out the USB. Go to the screen where you select "new game", "load game" etc. and put your USB back in. Now do the above steps. That should fix your achievement, at least it did for me.

Mission 1.1 or later – Can be glitchy!!
You have to die at least 20 times and the difficulty setting must be set to ‘Hardest’. Simply start a new game, set difficulty to hardest, lives to 5 and continues to 5 as well.
As soon as the game starts, let yourself be killed using all your lives and all the continues until “Game Over” appears on the screen, unlocking the achievement.
Note: In case it didn’t, restart a new game with the same settings but this time choose 4 continues instead of 5.
