Once you defeat the first four (real) bosses. you will get this achievement.
The levels are: Pridemoor Keep, The Lich Yard, and then two out of three of: Iron Whale OR Explodatorium OR Lost City.

I don't believe the plains count towards this achievement as the Black Knight is not a member of the Order of No Quarter. Therefore, you would need to defeat two of those second level area bosses.

Story related. King Knight and Specter Knight are required in order to advance through the game, but as for the other 2, you can choose any 2 out of the set of Treasure Knight, Mole Knight and Plague Knight. The order in which you do them doesn't really matter in the grand scheme of things because you will have to defeat all of the Knights of the Order of No Quarter anyways to beat the game.
