Ok, so this achievement is semi-glitched, both in a good way and bad way.(THIS IS NOT A GUIDE TO GET THE ACHIEVEMENT, THIS IS A GUIDE TO UNGLITCH)
First things first, you'll notice some deaths don't count, that's the way it glitches in a good way.
However, I finished the game, every perfect, and no achievement. If this happens, just play "The Royal Tutorial", and you should have it.

Did you do it on Normal or higher?
If so, I would recommend that you double check that you have every level perfected, and if that doesn't work, unfortunatly I don't know what to say(If you haven't already done the Grimm achievement, I would suggest that you go on and do that one, and maybe it might pop up later)

i to have seen this glitch, i know i have perfect on allmissions but acheivement didn't pop up. i have tried replaying the inro and "the royal tutorial" but this still didn't help in any way if anyone has any ideas drop me a line.

This achievement isn't too difficult, you will get most level perfects on your first try, then just use level select on Normal to finish them up.
If you fall down a cliff / hole, this does count as dieing and you will have to start the level over, so watch your footing.
Also you don't necessarily have a health bar, the more damage your take, your screen will start turning red when you are close to death, just hide / avoid damage until your screen returns to normal.

Sometimes this achievement glitches and doesn't pop.
Then you may spend a long time replaying all the levels perfectly on all the difficulties, and still not get it.
That's what happened to me.
The solution if it glitches is to delete your save game data and beat the game again, perfectly from the beginning on normal. Make sure you get all the levels perfect before you play the last level.
When the credits are done if it still hasn't popped (assuming you got the last level perfect) keep playing the game, play the introduction on hard, buy some things in the gift shop (like the Grimm difficulty), Some people even suggest replaying the end as well.
And then with luck the achievement will pop within 10 minutes or so of you beating the game.

I managed all levels flawless except for the Evil Queens Castle. I fell off 2 platforms outside with Puss & once with Shrek on the rotating platforms at the end of the level & these deaths didnt count.

Obviously easiest on Normal, simply go through every level without dying. If you fall into a hole or off the side, this counts as dying and you'll need to start over. The health works much like Call of Duty where the sides of the screen will turn orange and then red when you're losing health, simply avoid getting hit for a bit and it will go back to clear. Go through the game first on Grimm for that achievement and you should get some of these done, then use level select on Normal to finish them up.
