I did exactly this and didn't get the achievement. How is this missable at all?
Edit: 4 months, a trouble ticket to the devs and another partial walkthrough later I ended up using the debug commands on PC. Even that didn't work first, I had to delete all my saves from the cloud, then uninstalled the game from the console and installed it only on PC where I deleted my profiles and started fresh. After a few tries it finally worked.
Perhaps, this is hell.
