So, I have died well over 20 times on the same save file and it's just not unlocking. Not sure what I need to do to force it to unlock.

UPDATE: I have now died over 16 times on two completely different save files, one on Xbox, one on PC. I also deleted the save file on my Xbox to re-download from the cloud and that didn't change my luck either.

UPDATE 2: Tried a third save file, this time starting fresh rather than loading an old one. Rushed my way to the first enemy who could kill you, died 20 times, triangles filled. No achievement.

My achievement's bugged. I can't get it.

UPDATE 3: GOT IT!

Unfortunately what I had to do was manually delete both my on-console save and my cloud save. It was bittersweet, since that had my completed save file on it, but I thought, "I'm trying to finish my achievements before it goes off gamepass, what does it matter if I have my saves?" After starting the game again, fast tracking to the first enemy and dying 16 times, I loaded the save again, and it finally popped.

If it's bugged for you, just delete your cloud saves and hopefully it resets the counter.

I wonder if you have to do it during your playthrough and cannot load an old save? I swear I've died more than 20 times now after beating the game and reloading an old save in the hospital and nothing is happening.

I don't see the triangles in the diagnostic screen... or they are not working as intended.
