This event occurs in Room 112 of the Order Lair, almost right before the end of the game.
Approach Wheeler in the chair and you will be faced with two choices:
Y - Give him a medkit
X - No, he's to far gone
Simply press "Y" to give him a medkit and you will net the achievement
