This trophy will pop after fulfilling the following requirements:

Promoted to Sergeant: Fulfil the requirements for Lex Talionis .

Total playing time is 2 hours: You can keep track of your playing time in the Pause menu. Be aware that you need to actively play the game, as the time spent on menus does not count.

Find the weak spot on Matouschka: Be equipped with the Speed Up capsule and reach the last phase of this boss fight as you would normally do. When you reach it, go to the bottom-right part of the boss and use the capsule: time will slow down allowing you to shoot a rotating bar on the wheels. Destroy it to dispose of the boss and get this requirement done. If you forget to do this during your Story mode playthroughs, you can do it in Boss Training on Normal difficulty.

Finish any stage with a hit ratio over 85%: Kill 85% of the enemies in a level but this will come naturally so don't worry about it.

Destroy 50 ships: The ships this requirement are referring to, are the seaships in levels 2, 5 and 6. Submarines in level 2 seem to count as well. Destroy 50 of them to get this requirement done. You should get it before you are done with Story mode requirements, however, if you don't, you will either get it later in Arcade or Score Attack modes or you can grind the kills by stage-selecting level 2 in Story mode.

Destroy a boss with 90+ Seconds on the timer: If you do not get this naturally in Story mode, select Matouschka on Normal difficulty in Boss Training mode, choose the GE-22 Liberator ship with lv.7 firepower and equip it with a maxed out Reflection capsule and get hit as few times as possible throughout the fight. Unless you are terrible at the game, this should be extremely easy to accomplish.
