Yeah when you have her at low health an she starts screaming and is super vulnerable "DO NOT HIT HER" give it a couple of seconds an she will transform into her second form. [Achievement unlocked]
Also note that she recovers full health but is super easy to beat than her first form. All I used was the 2h sword to beat her. Just play it safe and when you know you can strike her run up to [hold RT] and press [X] and hit her again then just back off and repeat. You can use the spell that powers up your weapon to speed up the process
