The pond in the Fitz Tilus fight will remove poison, all you have to do is the poison build during the fight then roll in the deep water of the pond.

During the fight with Fitz Tilus you have to get poisened til the frame of your health bar starts to flicker and becomes orange. At this moment quickly roll to the water pond and achievement should pop. Only this way it did for me. Good luck!!!
