you actually don't need to create a graphic. Go to Edit Skater, then Merchandise, and then in T-Shirts theres a blank one that should be the first one.
Just select that and it will give you the option to add a graphic. Just do that and the achievement is yours!

go to skate.ea.com - log in - create a graphic and send it to game
when in game put on a pice of custom clothing, run down to the custom graphics at the bottom (numbered) and select one...
the achievement will be unlocked and you dont even need to wear that clothing :)
