How to unlock the Headway achievement in Skeletal Avenger - Definitive Guide

First, apply your upgrades to your skull to up the damage and have the Ram Horns helmet equipped which adds a lot of damage as well to your skull throw move (B button).
Then select and easy map on the quest board and try to gather perks that pertain to upping your skull damage. Now proceed carefully through the quest and try to avoid large mobs, throwing your head and picking it up again. Dash away when enemies are close. I had picked up a perk that applied necrotic damage to the floor when I dashed and that helped finish off some enemies when I was try to pick up my head.
As long as you don't touch the X button, any other types of kills (passive, environment, etc.) shouldn't negate the achievement. I got mine when I finished the quest (on the stats screen) without hitting the X button.
