Similar to the 8 kills with one skull throw achievement. Head to a quest in the sewers ("The Gutter" - 2nd area) and find some of those red sewer drains in the walls that spawn the scorpion enemies. Not all levels have them so keep questing until you find them.
Make sure to have fully upgraded your skull in the upgrades section (I had +3 over the max on mine when I got it) and most importantly, make sure to have the Magic Hat equipped since it explodes in a radius splashing damage in a circle once it makes contact. I wasn't able to actually acquire this until after beating the game and starting my New Game+. If you have any skull related perks as well it would help but not necessary. Also, I had fully leveled up Weapons upgrades and a powerful hammer for Melee.
Once in a sewer quest, find a room with the red wall spawns and let it spawn at least 25+ scorpions. I had probably this many plus some other enemy types bunched together. I kept running around in circles until I was sure there were enough enemies. Then I dashed into the middle of the pile and pressed B to throw my skull. It should hit immediately since you are surrounded and the circle of damage should take out many. What doesn't die from your skull, start smashing the rest with your hammer to clean up the rest. If you killed 25 in under 10 seconds, the achievement should pop right afterward. If not, let them respawn and try again.
