To get this achievement you have to ignore any NPCS trapped in cages though you can break the cage and still get the achievement as long as you don't talk to them also to clarify you can still have freed the NPCS that go to the castle after you save them and get the achievement

Do not break cells with NPCs throughout the entire race, if you break them, then restart the game by turning off the console and you will continue from the beginning of this room

Cells can be broken, the main thing is not to free the characters.
