This one is a bit silly, and I'm not sure if anyone knows totally how it works to be honest. However, I racked up tons of progress on this without trying, so I would not worry about it, especially since you have to play 25 games of each mode to 100% the game. You will have it by the end.
My suspicion is you get progress on this when a kill happens around you when you have less than 10% health. This could mean you either got the kill yourself or got an assist (assists are proximity based, you get them even if you didn't do any damage).
A couple notes:
-Assault is a good place for this, since kills are all happening in a small area (so you will get assists on everything) and no one can go back to base to heal, so you (and everyone else) will be running around with considerably lower health than other modes.
-There is occasionally dialogue when your character survives at low health. For example Zeus will say, "I'm getting too old for this" as you walk away with low health.
-Progress is tracked via Xbox Live so you can snap the achievements app by double tapping
to check your progress at any time.

You have to either escape or win a fight (defeat the enemy) within a certain amount of time while u have 10% HP or less. Tried it and got progress right away!

There are several ways to earn this achievement. Utilizing gods with leaps or escapes are the prime ways to exit battles when you have low health. Purchasing combat blink can also help a god without an escape get out of sticky situations. Through the natural play of the different game types you will earn this achievement most likely without realizing it.
