As Cthulhu, drive each enemy god insane at least once during a single match.
Torment, leading to insanity, is a new mechanic for Cthulhu. Via Cthulhu's passive, you can stack Torment on enemy gods. Each of your abilities will add at least one stack of torment when it hits an enemy god, and the last hit of your basic attack chain also adds one stack to an enemy god. On reaching 4 stacks of Torment, enemies are afflicted by Insanity (they start seeing and hearing weird things around the map at 3 stacks). You simply have to make every enemy go insane at least once in one game.
You can tell when an enemy god is insane as they will have a red/purple coloured aura around them (you passive meter will also show one eye open for each insane god near you).
Joust is probably your best bet for this, as you only have 3 enemy gods to send insane. However, can also be done in arena, with 5 enemy gods to send insane, or if you're reading this a little while after release, once Cthulhu has become available in ranked Duel, you only have to send your single enemy insane.
