To clear up confusion: Die 10 times in 1 level.

I recommend level 4.

You can get this and the can't you read achievement on level 1 at the start of the game.
There's a danger sign near the edge when you first wake up. Just throw yourself over the edge 5-6 times for two easy achievements.

Game is just buggy. Took 7 times to get the 1st Die in Can't you Read & another 6 for this one to pop.

I unlocked this after dying many times in one level. Now it could be I died a total of 100 times in the game, or 25 in one level. I'm not sure what is required to unlock, but it will unlock easily in a normal playthrough.

100 and 25 is far too much.
I did it on level 6. There's a checkpoint just in front of a pit of spikes. It unlocked after about having died 10 times in that level.
I only died 3 or 4 times in the levels before that, so that makes the most sense.
