In case you have not done enough lethal takedowns by the time you have finished the campaign, a good place to grind this out is in mission 8 “Rubble and Ruin” at the very beginning of the mission (starting location: City Limits).
When you reach the very first set of enemies, wait for the three enemies on the right side to disperse, then make a manual save. Takedown (
) the enemies in a counter clock wise fashion, starting with the enemy directly in front of the sandbags (then go to the general on the right etc.). You will get a total of 4 lethal takedowns. Reload your manual save and repeat.
If you make your manual save quickly enough, you can take advantage of the sound mask and sprint to each enemy which makes each run even quicker.

Lethal takedown are takedowns done with your knife, using the button. I have found them to be extremely effective in this game, and this achievement popped before any of the other weapon related achievement for me.
The best way to rack up kills is to use your minimap to identify where the enemies are (white circles), crouch down behind a corner, crate or similar and use your whistle to get them to you. You equip the whistle by holding down your weapon wheel
and you will find the whistle in the top right quadrant. Use it with 
. When they get a yellow question mark over their heads they will come looking for you and you can safely kill them from behind your cover. If you pay attention on the screen, you will get a prompt when they are in range but it is pretty forgiving. Remember that you can also use your awareness (hold down 
) to see enemies around corners and through walls.
You can also sneak up on enemies from behind by crouch-walking of course and you can even run up to them and knife them before they get their weapon up if you can time it right.

A lethal takedown is when you are next to an enemy and press 
to kill them. They can be unaware or alerted to you; either will count. If they are unaware, it will count towards Der Geist (15G).
If you are playing on lower difficulties, if enemies are close to each other, you can just go from one to the other and kill them all with a takedown.
A good method to get this out of the way is to create a save just before you kill an enemy(s) with a takedown. Once they are, just reload that save and keep repeating until the achievement has unlocked.

then navigate to Service Record > Medals > General and look under the ‘Combat’ heading to see your medal progress.

You don't need to stay stealth for that, however during combat the enemies might block your kill, so it's best to come from behind them. You can use the save/reload feature to quickly farm kills.
