I did this as the first achievement in the game because… well, I regret nothing.
It was very first part of the mission, ‘behind enemy lines’. I was on top the tower. Using the ladder as 12 O’clock, turn to 3 O’Clock, just past a smoking chimney. There is a guard standing on the upper floor of a roofless building, just pacing back and fourth on a predictable route. I shot from the prone position between the broken bricks of the tower.
I recommend saving just before going for this so, you can reload if needed. I also recommend holding the empty lung button (RB
).

Rich, Walker At the beginning of the 'All or Nothing' mission, there's a Jager Sniper in the window around 200 metres away. I managed to accidentally explode his nuts from that distance, so if you're actually trying to get this, it should theoretically be easier. Generally, if you try to aim long-distance shots around the low crotchal region, you should eventually hit ballbag and unlock this. Good luck!
Rich, Walker, and, bLaKgRaVy This may be earned through natural progression, but you just need to make sure that your target is at least 100 meters away. It is suggested that you survey the area with binoculars and tag any enemies that are visible. Focusing on an enemy will reveal its distance in relation to you. Choose your target then create a save in case you don’t hit the correct area.
With your rifle, as you’re holding
, zoom in fully and aim for the crotch area then press 
to focus your shot. When the reticule turns red, take the shot. You do not need to see the x-ray cam; as long as it registers as a testicle shot, the achievement will unlock.
In my experience, the enemy needed to be facing me for it to register as a testicle shot. If you don’t hit the right organ, just reload your save or try on someone else.
Note: You will not have a reticule on ‘Authentic’ difficulty.
