Guardian Angel
-Rescued the friendly ("From Out Of Nowhere")

You will unlock this in Act 1, mission 2 when you help your team to storm the building and save the hostage.
You have to shoot the last man holding the hostage before he backs away and kills him.
(Thanks PuckJunkie for confirming that this is missable)

You make this sound like it's unmissable... if you wait too long to take the shot, or if you fire and hit the friendly, the mission continues with the friendly dead. You have to kill the gunman before he backs up all the way to the balcony's edge, and you can't hit the friendly when you do it. It's a fairly difficult shot if you're playing on the hardest difficulty level.

I didn't actually know that. I always thought the mission will fail if you don't shoot him. I edited the solution thanks!

This is a story related trophy and is un-missable. Please see below for more information:

In the 2nd mission, From Out Of Nowhere, you will be providing overwatch for a squad of soldiers in the beginning of the mission. At the very end of the overwatch segment, an enemy will be using a hostage as a human shield but has his back to you. Kill the enemy without hitting the hostage and the trophy will pop.

STORY RELATED CANNOT BE MISSED!

This is done in Act 1 Mission 2. You will be asked to clear out a mansion so your team can move in and save the hostage. Once your team rushes into the mansion, your teammate will tell you that the hostage is held at gun point. The enemy will start to walk out to the top of the mansions roof, aim and successfully shoot the enemy in the head to free the hostage.
