To hold you breath you press in the Left Stick. Kill 50 people while doing this and the achievement is yours. You can combine this with the Night Vision achievement as well.

Holding your breath is done by pushing and holding . On all difficulties but expert it slows down time, and it reduces scope sway on every difficulty. The majority, if not all, of your kills should be done while holding your breath as you should take any advantage you can when trying to kill the enemies. There is no way to track this number, but since there are an abundance of enemies in every mission you will more than likely earn this on your first playthrough.
