You will get this many, many times during your playthrough of the campaign. You really don't need any specific instruction. There will be plenty of opportunities when your enemies will have no idea where you are and you'll have plenty of time to line up a shot on their heads, hold your breath, *bang!*

When you aim with the sniper you probably think you shall put the Middle on the head but that´s not true... When you get really close to the enemy with the middle of the scope, a little red cirkle will appear.. aim that cirkle att the head in slow-mo and you´ll get the achievement before act 1:) Good luck

This trophy has to be done in Singleplayer. Keep aiming for the enemies heads, as headshots give you instant kills. You should definitely be able get this trophy after the first few chapters of the Ghost Warrior campaign.
