The achievement is to finish the game with no character ever dying.
The "guide" is really more of a pro tip. Difficulty settings don't void achievements so my recommendation is a custom difficulty playthrough with all sliders tuned to your favor. Enemies will not be able to hit 99.9% of the time, so there is no danger of dying and you can kill off enemies quickly.

Fun Fact: You can get this achievement on the palace of ice DLC, but not any other.

Question, does "dying" in this case mean on the floor making death saving throws, or does it mean actually requiring a revivify or the like after failing 3 death saves?
