How to unlock the Silver Medalist achievement in SONIC THE HEDGEHOG - Definitive Guide
To unlock this achievement, you must find every Silver Medal in Sonic, Shadow, and Silver's respective ACT and HUB areas, as well as the final episode, End of the World. This is the single most time consuming achievement in the game excluding Legend of Soleanna and Gold Medalist.
The other solution listed here says that the medals make a noise when close. They do, yes, but they can be difficult to hear depending on other sounds present in the game. If you're going after this achievement without a video guide, you can rectify this by going to the in-game Options menu, turn the BGM off and the SFX up. This makes the medal chime much easier to hear.
Also, try to defeat all of the enemies in a room when you're searching, as some medals only appear when they're all killed.
If you plan to use a video guide, I recommend ShadnicK826's YouTube channel. He has video guides for every single medal in the game and goes through them in quickly and simply. A link of one of his videos is included in the bottom.

Just got this, and I don't think I ever could have without this guy's videos -- they're very easy to follow, good quality (except the first few Sonic ones), and sometimes show off amusing bugs in the game. :) Since I didn't go for these while playing through the main story, I ended up doing each stage as I was working on the S-rank, and these videos also gave me some good shortcuts and routes to use in going for those, too. I was able to S-rank all of Shadow and Silver's stages just using these routes, and I've got a whopping three S-ranks remaining for Sonic (though I may have to look up actual S-rank videos for them because of incredibly meager time bonuses).
I just want to warn anyone going for this that it will pop as soon as you collect your last medal (and hang for a second, like this game likes to), but you need to finish that stage in order for them to save and earn your gold medal. I assume anyone going for this is going for the gold medals as well... :)

You need to collect all silver medals in the game. This can be very time consuming, as there are many silver medals throughout the game. Sonic has 130 medals, Shadow has 120 medals, Silver has 120 medals, and there are 10 medals in the final mission. There are medals in the town areas and in act missions. A great guide for all the silver medals is found at the bottom of this guide.
