Simply boost your Werehog Combat level up to 5.
The quickest way to do this is to complete the Werehog levels and collect as many of the yellow diamonds as you can from defeating enemies. Although any EXP earnt from Sonic's levels can also be used on Werehog Sonic.

By "Werehog level" it means Sonic the Werehog's Combat level. For this achievement you will need to raise your Combat level to 5.
