A first attack is done by placing damage on your opponent first. This will come naturally and you cannot miss this just play the game and go for the other achievements.

Chances are that this will unlock naturally if you are going for all of the achievements. If not, I would suggest characters with a long range such as kilik using his Y+Y combo or Nightmare / Siegfried with their forward+Y attacks. Any questions feel free to ask/ message me.
