There are 21 total toilets in the game. None of them are missable. You can find a text list of their locations HERE, and the video below shows them all as well. To master a toilet, you will need to complete a short QTE mini-game. The number of stars is supposed to relate to how difficult it will be to master, though it is actually completely random which QTE sequences you will get (and they will change if you fail). You could have a 4-star toilet with all easy QTEs or a 2-star toilet with all hard ones. Most are simple, such as holding the thumbsticks in different directions or pressing a few buttons at the same time, while others can be quite difficult if you're not very ambidextrous, such as moving one thumbstick up and down while moving the other side to side (kind of like rubbing your stomach and patting your head at once - my brain can't process that).
You have a specific amount of time to complete all the stars/QTEs, and between each one you'll have a small amount of time to "gas up" which adds some time back on the timer (indicated by the green bar next to the stars), and this action is always the same (rotate counter-clockwise and clockwise). Once you complete each of the stars you'll need to finish the mini-game with another QTE, though this part is not timed. You'll see a green check mark when you approach a toilet to show you've mastered it. Since you'll need specific skills to access some of the toilets, it's best to finish this after the story is complete, though feel free to grab any you come across along the way.

3. When it begins just ignore all the different buttons and press M or RB and hold it long enough, the yellow bar should fill and voila.
It is a lot easier using this method.
There are a total of 21 toilets in South Park and each one has a star level that determines how hard it is. To master the toilet you are using, you must achieve the max amount of stars possible on it before time runs out. When you find one that you can use, press to interact with it. Once you've assumed the position, you must rotate your analogue sticks until you focus your power until it says hold above both sides, then hit to begin the pooping mini-game. During the mini-game, you will have to complete a series of challenges that involve some combination of analogue stick movement and button pushing in order to fill your gauge on the left side of the screen up a star level, all while your time bar starts to go down. When you reach each star level, you will be giving a short period of time where you need to rotate both analogue sticks in a counter-clockwise direction quickly to build back up your time gauge. Once you fill your star gauge all the way to the top, you will then do one final one to release the poop into the toilet. Once you have mastered all 21 toilets, the trophy will pop.

I found toilets ranging from 1-3 stars were fairly easy but could not master any of the 4-star toilets. If you are also struggling with any of the toilets or you just want to get them done quickly then go to options menu and turn on the mini game assist. This makes it so you simply just need to hold R1 during the toilet mini games and you'll see the bar fill itself up to the top without needing to follow any of the commands on the screen.
The mechanics are very simple: in the first stage of the round you need to perform QTEs, filling the left gauge, and in the second - on the contrary - you need to prevent the right gauge from emptying “Get on the gas!” And so on in each round, the number of which, as mentioned above, varies.
After completing the mini-game, do not forget to check whether the toilet sign has a “tick” - this will mean that “peak skill” has been reached.
