I think this won't unlock of you okay the demo first and continue the game later. That is what I think happened to me. Might unlock on next playthrough

This can be almost as soon as you gain control of your character. Head to your bathroom and play the toilet mini-game. Fairly simple, pressing a few buttons within a time limit.

You can earn this achievement as soon as you gain control of your created character. Simply match the thumbstick prompts perfectly and you will get the star required for this.
