All you have to do is find and play the Monty guess the right box challenge. You do not have to win just play 20 times. After 10 times he will give you a new title.
Find one of the travelers in one of the areas and he will give you a medallion and tell you to find his friend in a certain area. Go find him and he will give you a map patch for that area. Load those in your bag and when you drop in the first area pull up your map and see if you see a circus tent(round icon). If not there just exit out and restart a new game. And another map will respawn.
Monty will and can show up in any of the five areas.
Vineland is the one are you start in.
Golden wood area is to the east.
Ice world is to the north.
The desert is to your west.
The Blue green acid area is to the south,

i would recommend to do this at the beginning of the Game or after beating the Game, start a new Game. I beaten the Game on normal and visited Monty only 9 Times (had a great run defeating 3 Bosses first try) and after unlocking all areas it is a "pain" (just takes longer) to run far away to reach Monty. At the beginning he is always in the start area and after you get the map you see where he is. So there is no need to restart if he's on another map.

Monty can be found in the building shaped like a pirate ship, which will appear at random on the map. He runs a game where you pick one of three chests for a chance at a prize and each time you visit him, he marks your card. Eventually you will upgrade to a Gold Card, which should unlock the trophy.
