To recruit Sugata, you need to complete the mission "Terror In The Tundra".
Mission requirements: Recruit Mayura and Lamia; complete the mission "Unidentified Monsters 2".
Recruitment requirements: Defeat all of Sugata's minions and then Sugata; Mayura and Lamia do not retreat.
Once you have unlocked the mission, complete it without Mayura and Lamia retreating. This mission can be a bit tricky due to the map layout. First defeat the group of minions nearest you. Then either have your party cross the crystal bridge and defeat the second group, or draw them to you by getting close enough. Sugata will attack you from distance if you get in range, but she will not come after you until you get up on the raised area she is on.
The achievement will unlock after you have saved your game.
