You need to lead all the refugees from their temporary camp in Farlon to Vsesvet. They need to ensure that none of the 46 refugees die. The route is long, you need to lead them first through Farlon and then through the Iskander Woods. The trophy will drop when you talk to Clara in All Light. I recommend that before talking with Clara, who is launching the caravan, you make a save, and then run the route with your army and clear out all the enemies on it. Then there shouldn't be a problem. Otherwise, you may have problems on the final section of the road through the Iskander Forests, there will be a large crowd of bandits who, even on the minimum difficulty level, kill refugees in a couple of shots.
