When in Greykeep Prison, pick up the Mysterious Key right above the chest where you get your armor back. Free the 5 prisoners in the north of the prison. They are right next to the north west exit. I exited south east with them, and told them they were right to fight the guards (they followd me afterwards). Fight your way to the Tower Exit. When you reach the end of Greykeep Undercroft you get the achievement.
