When escaping Greykeep you will meet Alistair in the Undercroft. Chose the following dialogue options to convince him to let you go.

Definitely missable if you are helping all 5 prisoners escape for the other achievement. Fortunately the manual saves are available if folks are remembering to use them.

You need to convince Alistair to let you through. In order for the necessary dialogue thread to become available during the dialogue with Anselm in the previous chapter, you need to refuse to carry out the order to kill the villagers. Moreover, during a prison escape, you cannot save the prisoners or you must send them to free swimming after the fight at the bridge, when you have such an opportunity. Otherwise, they will start a fight with Alistair, and you will not be able to get this trophy. So this trophy is not compatible with the "Escape Artist" trophy.
